NTSB: Widespread problems factor in boy's death

The National Transportation Safety Board is pointing to lax oversight by the Coast Guard as a factor in the 2009 San Diego patrol boat crash that killed 8-year-old Anthony DeWeese, according to a report released Tuesday that also concludes that the Coast Guard boat pilot was driving far too fast – as fast as 42 knots – for the crowded conditions on San Diego Bay that night.

The federal board discussed its findings Tuesday morning at a meeting in Washington, D.C. The full report will be published in coming weeks, according to officials.
