Sailracing skipper was not wearing life jacket

The Australian captain of the maxiyacht Shockwave was not wearing a life jacket when he was swept overboard as his vessel crashed onto rocks in an accident possibly caused by human error or faulty navigation equipment.

One of the surviving crew described rough conditions that pummeled the 79-foot yacht, describing a wall of water sweeping the skipper and navigator overboard.
